Longer Term Needs
In order to support our pupils with long-term medical needs in school, an Individual Healthcare Plan (IHP) is completed. The aim of these plans is to ensure information is easily and effectively communicated across the school and to put in place any necessary steps to ensure your child’s health and safety.
Pupils with long-term medical needs are defined by one, or more, of the following criteria:
1) The medical condition affects their participation in school activities.
2) The pupil is required to take medication in school for their condition.
3) The pupil requires extra care and support for their medical condition.
4) The pupil may have periods of absence from school due to their medical condition.
5) The medical condition has the potential to be life threatening.
If your child is new to the school or has a new long-term medical need, please contact the school office..
Short-Term Needs
Pupils who need medication administering in school for short-term medical needs, such as paracetamol or antibiotics, do not need an IHP, but a Medication Administration form must to be completed by parents/carers.
All medicines must be brought to the School Office and handed over with the completed form, please do not send your child into school with any form of medication in their bag.

Our school uses Medical Tracker to record, manage and communicate information about pupils’ medical needs.
Medical Tracker is a secure, online system that helps us ensure that pupils with medical conditions receive the right care at the right time while they are in school.
What Medical Tracker Is Used For
Medical Tracker allows staff to record and monitor:
• Medical conditions and care plans
• Medication administered during the school day
• Allergies and dietary requirements
• Accidents, injuries and first aid treatment
• Parental consent and medical documentation
This information is shared with relevant staff so that children are supported consistently and safely throughout the school day.
Administering Medication in School
Parents and carers must complete the appropriate consent forms before any medication can be given in school. All medication administration is recorded on Medical Tracker, including the date, time, dosage and the member of staff who administered it.
This helps us keep accurate records and ensures clear communication with parents and carers.
Communication With Parents and Carers
Depending on the nature of the medical event, parents and carers may receive automatic notifications via Medical Tracker when first aid is given or medication is administered.
This ensures you are kept informed and reassured about your child’s wellbeing while they are in school.
Storage and Security
Medical Tracker is a secure system that complies with data protection standards. Access is restricted to authorised staff only, and information is handled in line with our data protection and safeguarding policies.

ALLERGIES
If you are aware of an allergy that your child has, please contact the school office immediately to ensure staff are aware of this medical information and take appropriate care and precaution.
Please remember, allergies and tolerances are very different. If there are any specific dietary requirements, please inform the school office or your child’s class teacher as soon as possible.
Children who require the use of an Epi Pen in an emergency must have TWO, IN DATE Epi Pens in school at all times and must have a IHP, as outlined in the above section about long-term needs.
PUPILS WITH ASTHMA
At our school, we use Medical Tracker to support pupils with asthma and to ensure their needs are managed safely and consistently.
Recording Asthma Information
Medical Tracker is used to record key information for pupils with asthma, including:
• Diagnosis and severity of asthma
• Individual asthma care plans
• Known triggers
• Inhaler type and dosage
• Parental consent for inhaler use
This information is shared with relevant staff so that children can be supported appropriately throughout the school day.
Inhalers in School
Parents and carers are asked to provide an in date inhaler for school use, clearly labelled with the child’s name. Consent for use of the inhaler must be given by completing the required medical forms.
All use of inhalers during the school day is recorded on Medical Tracker, including the time, reason for use and the member of staff supporting the child.
Where required, schools may also hold an emergency inhaler in line with government guidance.
Supporting Pupils With Asthma
Staff are trained to recognise the signs of asthma symptoms and respond promptly. Children are encouraged to understand their condition and, where appropriate, to carry and use their inhaler responsibly.
Medical Tracker allows staff to monitor patterns in inhaler use so that any concerns can be identified and shared with parents and carers.
Communication With Parents and Carers
Parents and carers may be notified via Medical Tracker when an inhaler is used in school. This helps to maintain clear communication and ensures any follow up actions, such as replacing inhalers or arranging medical reviews, can be taken promptly.
Keeping Information Up to Date
It is important that parents and carers inform the school of any changes to their child’s asthma condition or medication. This ensures that Medical Tracker records remain accurate and that the correct support is always in place.
